The Director of Case Management Post Acute Services is accountable for coordinating, developing, executing, monitoring and evaluating all case management activities. He/she is responsible for providing oversight and direction of clinical and non-clinical services specific to Case management and Social Services within Post Acute Services. Case Management encompasses utilization review, resource management, coordination of care, transition/discharge planning, and social work services across the episode of care. The director assumes a leadership role within the interdisciplinary care team and facility. The Director is responsible for all aspects of the Utilization Management Plan and promotes cost effective quality outcomes that are monitored across the care continuum. Through effective managerial and educational direction, in the areas of Case Management operations, resource Management, timely movement of patients through a continuum, DRG/RUG financial analysis activities provides hospital-wide leadership. Frequently Interfaces with, Senior Management, medical staff, quality, admissions, marketing, ancillary services and nursing in a communications and change management capacity. Responsible for oversight to ensure all Case Management activity is in compliance with all state and federal regulations.
