  Enterprise Health Staffing, LLC is a national care management company that provides a team of professionals to assist with keeping senior citizens out of the hospital and emergency room. Enterprise Health Staffing partners with insurance companies and medical groups to provide their chronically ill clients a personalized plan of care to assist them to stay in their home. Our Care Managers are licensed, clinical professionals that have been trained one-on-one to accomplish the client’s goals for a number of in-home care management programs. At the direction of our institutional clients, their clients are assigned a licensed nurse to provide ongoing assessments and care management. Our Care Managers (licensed RNs / LPNs / LVNs / Social Workers) act as advocates and navigators to assist members/patients with accessing benefits and resources. They are the client’s coach for resolution regarding disease management, self-care, medication reconciliation (to help avoid medication confusion), and identification of gaps or barriers to care (without providing direct care themselves). We accomplish this specifically by..... A referral is made from the Primary Care Physician, or the family through insurance companies or medical groups. Enterprise Health Staffing will assign a Care Manager to the client. The Care Manager contacts the client within 24 hours of receiving the assignment to explain the in-home care management program and to schedule the first visit. Upon the first visit, the Care Manager shares program documents with the member. Weekly face to face meetings are scheduled for the convenience of both the Care Manager and the member. As the member’s needs change, services will be modified to meet an appropriate level of care. Show less
