      I interned at the San Francisco Department of Public Health's (SFDPH) Permanent Advanced Housing Clinical Services Team (PHACS). PHACS is a SFDPH funded team who's goal is to provide mobile medical services to supportive housing projects throughout San Francisco. The populations who live in these state funded projects are generally very vulnerable; usually they are formerly homeless people with medical conditions, phycological issues and addiction. My role at PHACS was to shadow the mobile health care team as they made their rounds around the projects. I also provided services to the team, such as: picking up medications for patients, accompanying patients to appointments, and aiding team members when necessary. My work at PHACS cumulated with a data collection project to improve the services that PHACS offers. I created a survey and analysed data which improved services. Show less
