By following the Care Act and promoting Good Lives, I promote and help service users to maintain their independence, as much as possible, in the community. Working as part of a multidisciplinary team, I work in partnership with an array of professionals, all of whom have different views, perspectives and processes. I complete Care Act assessments, Safeguarding Concerns and/or Enquiries, Mental Capacity assessments and continuing healthcare assessments; as well as other assessments and referrals. Assessment can be a fluid process due to changes in the adult or pressures from hospital and can change at the point of discharge; therefore empowering adults by informing them of their rights, for example their right to make wise/unwise decisions is critical. Duties include, but not limited to: • To assist with discharging service users under annex G of the Care Act 2014. • To support adults who are ready to leave the hospital through the discharge process. • To advocate the rights and entitlements of adults with or without capacity. • To work in partnership to ensure adults have access to the support most appropriate to their needs; working with care providers, CCG, SPT. • To ensure a timely discharge from hospital and prevent delays. • Collect information to inform practice. • Participate in MDT – to agree plans. • Attend DST meetings. • Utilise community resources e.g. technology enabled care, care co-ordination, mental health teams and GP’s. • Work alongside community teams to ensure good outcomes. • Advocacy for those without informal support . • Undertaking Safeguarding Concerns and progressing to Enquiry, where necessary. • Undertaking Care Act Assessments. • Completing Mental Capacity Assessments and making Best Interest Decisions. • Safe transfers through good record keeping and communicating ongoing concerns.
