Duties: Facilitate planning process and develop the Individual Program Plan (IPP) for a case load of 101 clients, oversee and implement the IPP, monitor and evaluate the services provided through the plan, evaluate the effectiveness of the IPP ensuring that appropriate community resources are made available to the client; conduct annual reassessment of client’s developmental status in conjunction with interdisciplinary team; monitor vendor services and verify that services are delivered in accordance with the purchase of services, and support the implementation of the client plan including generic and natural resources and evaluating when a need exists that can only be met through NBRC authorized funding; Implement NBRC policies and procedures; keep manager informed of the status of case loads and unmet needs in the community; advocate on behalf of clients and protect their rights under the Lanterman Act and ensure that they receive those services and benefits which they are entitled; assist clients and families in obtaining essential diagnosis and evaluation from appropriate practitioners and secure written reports and other information which assesses and interprets the client’s situation, monitor client’s health and safety, refer clients to day programs, and assist in residential placement of clients.
