In my current role, I autonomously complete targeted health assessments for 10-20 patients per shift and generate individualized wound treatment plans for those with abdominal stomas, fistulas, drains, pressure injuries, and continence needs. I am highly skilled and proficient in completing bedside procedures including wound debridement, suturing, I & D, and biopsies to support effective medical resource utilization and cost-effective care in a timely, professional manner. Like similar roles, I consult and confer with advanced care specialists to facilitate referrals, when needed, to improve outcomes. I strive to empower my patients and their families, through education, to understanding their medical conditions, treatment options, and to make informed healthcare decisions. I provide timely, detailed, and complete clinical notations to the EMR in compliance with organizational policies, local, state, and federal healthcare mandates. Selected Contributions & Accomplishments: Influential in growing the practice by more than 66%; organization was established with 1 LTAC and 2SNFs and has expanded to now include 4 SNFs. Instrumental in the delivery of telehealth services and the implementation of telerounding as a means to ensure patients timely access to quality medical care with minimized risks of further disease exposure. As I transitioned into this role, I independently provided comprehensive care for 13-36 newborn, pediatric, and adult patients with varied health conditions in a fast paced, outpatient clinical setting. I performed history and physical examinations, and leveraged observations with diagnostic results to generate individualized care plans utilizing pharmacologic and nonpharmacologic interventions. I interfaced closely with multidisciplinary provider to refer patients for higher level or specialized are, when needed. I accurately documented all clinical notes in SOAP format to support effective management and continuity of care. Show less

Family Nurse Practitioner

Apr 2018 - Nov 2018
In my first advanced practice role, I demonstrated proficiency to interview patients to obtain medical histories, completed thorough health assessments, ordered and interpreted diagnostic labs and/or images to accurately diagnose acute and chronic conditions in patients across the lifespan. I successfully completed up to 6 clinic visits per hour in this private practice setting. To effectively manage conditions and promote positive results, I prescribed diverse treatment modalities and monitored responses to care modifying plans and facilitating referrals, when needed, to improve outcomes. During this experience, I provided well-childcare, administered immunizations, school and sports physicals. I offered, to each patient and family, the education and resources needed to understand anticipatory guidance, treatment regimens, preventative care and healthy behaviors. At the end of each appointment, I documented all exchanges to the corresponding medical record to comply with rigorous standards of care and practice. Show less
