I have worked for 6 months as Community Coordinator in THORNBURY COMMUNITY SERVICES in Bristol. Care coordinator is the initial point of contact for nurses and clients and responsible for placing nurses and healthcare assistants into commissioned care package bookings throughout the UK working alongside a team that are committed to making a difference to the lives of individuals and their families. Core duties: Acting as an initial point of contact for community customers, fulfilling routine and complex requests, responding to inquiries from clients and field staff, performing routine and complex transactions and customer care activities. Preparing timely ROTAs for families, and make them aware of any change, well before or 11th hour. Offering deal breaker, Bonus to fill that urgent shift. ROTA is shared with family on written and confirmation basis. After booking every shift, written and verbal confirmation from family and staff is must and recorded. Searching suitable staff for the client if anyone pulls out in an uncontrolled situation, using all possible sources, like been before team members and outsourcing ADHOC staff with different companies. At this stage, clinical hand over is very crucial and playing pivotal role in providing safe and outclass health care plan. It is duty of coordinator to liaise with clinical lead and case manager to handover clinical aspect to a new staff. Sometime shadowing is must. Carrying out timely clinical assessments with the help of Clinical lead, and make suitable adjustable in care plan, that lead to find the most suitable staff for the package. Providing guidance and support to field staff working in community placements. Providing knowledge to colleagues, clients and field staff.
