  My responsibilities were to monitor and coordinate state-funded services for individuals with developmental and/or intellectual disabilities. I was responsible for maintaining a caseload of individuals requiring monthly phone contacts, quarterly home visits, annual reassessments and planning meetings. I wrote Comprehensive Plans of Care, calculated annual budgets for each individual in my caseload as well as calculated revisions and submited to the state for approval. I monitored in-home and Day Habilitation staff and service delivery to ensure that services are being implemented in accordance with Medicaid, OCDD, and DHH regulations. I maintained confidentiality of client’s records and information based on Medicaid and HIPAA requirements and Advocate and assisted clients with obtaining appropriate Medicaid supports and services.
