The Queen's Medical Center, located in downtown Honolulu, Hawaii, is a private, trauma level one, non-profit, Magnet accreditated, acute medical care facility. It is the largest private hospital in Hawaii, licensed to operate with 505 acute care beds and 28 sub-acute beds. The medical center has more than 3,000 employees and over 1,200 physicians on staff.Key focus areas and accomplishments:• Facilitate coordination of care across the continuum for selected complex patients; infectious disease, specializing in general surgery and cardiac specialty.• Case finding and assessment, patient/family education and advocacy, resource management and discharge planning in an acute care, long-term care setting, or community setting.• Based on established guidelines (Miliman Care Guidelines: MCG, Interqual), identifies and re-directs care and treatment to ensure appropriate care and cost effective outcomes; ensuring appropriate level of care has been determined on admission and regularly throughout patient stay.• Identifies patients with social, financial or medical issues that requires case management efforts, and completes initial assessments in order to ensure efficient care and a safe, timely discharge.• Provide clinical information to payors when requested to ensure continued authorization for the stay.• An active committee member in the Nursing Practice Council (NPC) providing feedback and insight on contents and areas involving case management and discharge planning.
