  As RN Admission Coordinator, I provide a first point of contact for patients and families that are referred to our hospice by their physician. Most of the time we are dealing with patient's and families that are terrified and confused after hearing the word "hospice." I take the time with them to explain the full meaning of the referral and hospice benefit. Then I assess the patient and the information sent by the physician's office to ensure that the patient meets the Medicare guidelines for admission criteria. Once they are admitted, I tailor the plan of care for the individual needs of the patient and their family. Show less
