Clinic and Outreach Case Manager

Help homeless patients navigate their social needs such as housing access, financial support, legal concerns, substance use treatment, and other needs. Provide both clinic-based and outreach support. I'm also an active member of the BHCHP Equity and Diversity Committee and the Immigrant Health Committee. My day-to-day experiences are diverse - I'm helping one patient (a patient who speaks no english, has no income, and has no passport) find a way to get to Cameroon to visit a sick relative; other patients I help are working towards permanent housing, employment, and financial stability; and many patients need help accessing food, clothing, and substance use treatment. I work with patients in both English and Spanish
