      Initiate and maintain contact with homeless TAY (Transitional Age Youth) who have severe and persistent mental illness (SPMI) or a serious emotional disturbance (SED), and who have been determined to be at risk for long term homelessness. The focus is to provide services where the client is located at the time of interface. This may be on the streets, in shelter or transitional housing, clinic or office and eventually, in their own housing unit. The objectives of the program are to engage these persons in treatment and place in permanent supportive housing in the Hollywood and surrounding areas. The program is funded by DMH and is a 2 year grant. Primary duties include: • Street outreach as a team to initiate contact, develop rapport, assess for mental illness, co-occurring substance use and other health care needs in the community until linkage and services are accepted. • Make initial psychiatric and psychosocial assessment and make recommendations for enrollment in the program. Evaluate for a qualifying mental illness diagnosis or SED and make provisional diagnosis. This will include level of functioning, support network, adequacy of living arrangements, financial status, physical health, level of self-care, medication and treatment history and compliance. • Evaluate for immediate needs such as crisis intervention, medical attention, showers, clothing, food, emergency shelter, etc. • Assure access to local resources, including psychiatric and medical care, transitional housing or shelter placement, rehabilitation for drug/alcohol services, socialization activities, providing transportation and accompanying the client when necessary. Communicate regularly with other treatment providers. • Assist enrolled clients with housing applications, search and support once housed to ensure successful transition from the streets to permanent housing. • Maintain DMH charts in compliance with funder requirements.
