  I work to facilitate conferences to the best of my abilities by remaining culturally competent and working to undo my own implicit bias. I connect ACS and service providers of our agency (Supervisors, Case Planners, and Parent Advocates) with the families and their support systems to complete conferences.I set expectations to minimize group conflict during discussion and dialogue. Our family team conferences consist of collaborating with families to identify strengths, speak about any concerns or areas of support that the family may need, and co-designing an action plan with the family we are serving. This role is important in building consensus so that all participants may dissolve confusion, provide clarity about the purpose of our agency's involvement with families, hold one another accountable as we proceed to work with one another in successful goal planning, and so that services may be rendered with each client's right to self-determination.I am also responsible for making sure that all parties are provided with written documentation to reference the contractual agreement at the end of conferences.   I work in a preventive service division within the agency of Seamen's Society for Children and Families. During my time as a Case Planner, I was able to serve families that needed extra support in dealing with IPV (Intimate Partner Violence), unaddressed mental health needs, and substance abuse. Our agency connects these families to their community resources, provides financial supports when it is required, works closely with ACS Caseworkers, monitors home environment for safety and risk, and refers our clients to necessary or court ordered services. I was offered a role as Family Team Conference Facilitator, which inherently relinquished my role as a case planner. Show less
