Work closely with community partners and healthcare providers to support community health and school based health service programs for low income families, assist in health screenings in the schools as well as public health screenings at community events, and health fairs, ensure the proper maintenance of student health records, screenings and services, immunization records and medical authorizations forms as required, conduct targeted outreach efforts in the schools and community to families at risk, including those who are uninsured, underinsured and/or not connected to a medical or dental home or other community resources, provide basic health education to families on a range of topics including, oral health risk, dental disease, and other important health topics, provide education and referral assistance in obtaining affordable health insurance coverage for children, connecting to a primary care provider and obtaining urgent care and follow up services as needed, help families address and overcome access and cultural barriers to health care such as language, transportation, scheduling, child care resources. etc., assist families to develop the necessary skills and resources to improve their health status, family functioning and self-sufficiency, collect and accurately enter data and statistics into a computer program to document client interactions, case management notes and other required documentation, utilize basic computer applications to schedule, communicate and track community health outreach and promotion activities, create outreach materials for various programs, provide in-person and written translation, perform other duties as assigned.
