Participate with Director of Residential Services in the hiring, evaluation and termination process of direct care staff. Conduct intakes, evaluate and develop individual self-sufficiency plan in collaboration with DTA, to address critical short-term needs.Assess and monitor progress towards goals and compliance with program rules and regulations.Provide linkages and coordination with co-worker, DTA staff, Housing Search Vendor, and DSS including case conferences and or case review.Conduct home visits to participant's families at lease (1) one-hour per week. Additional weekly visits may be required to address problematic or household maintenance issues.Participate in weekly supervision.
