At Life Choices, we work with people from 16 – 60 years-of-age. Working in a person-centred way, Life Choices is an open-ended project that will help you to: - Access ‘main stream’ society - Increase your confidence - Increase your independent living skills - Develop your life skills - Plan and achieve your life goals. You will be appointed a specialist and dedicated Life Choices support worker/ life coach who will meet with you every week for an agreed number of hours. Your support worker will: - Help you to identify, plan, review and DO activities that you are interested in - Act as a life coach and mentor to you - Support and encourage you in accessing the activities that you want to do - Act as an advocate for you, if required.
