Those resources help support home and community-based service development and delivery. I have held primary responsibility for fundraising activities. • When COVID-19 came to Minnesota, we increased home and community-based service delivery by 39% from 2019 to 2020. Services focused on supporting the health and independence of older adults, with a particular focus on addressing social isolation during emergency stay-at-home orders. • Our investments in a broad range of home and community-based services totaled over $31 million from 2018-2021. • We are focused on addressing inequality in access to services that improve health outcomes and allow older adults to live independently in the community of their choice. In 2020, more than 40% of the individuals served by Trellis’ Older Americans Act program were from communities of color. The Corporation for National and Community Service is a federal agency whose mission is to meet community needs through our AmeriCorps and Senior Corps programming. This work includes nonprofit, government, and tribal partners who engage citizens of all ages in national service.
