This role is designed to aid patient discharge into the community or admission into the hospital. The APU role focuses on early targeted assessment and intervention of patients over 75 admitted to the hospital. Depending on their presentation, they may require short-term hospitalisation or transfer to a ward. This role has been in a development phase and progressed to a consolidated service; the team has won awards for its positive effect on the patients and hospital occupancy rate during the year.
