As a HUD Case Manager, I provide case management services to individuals who have left homelessness and now reside in permanent and/or temporary housing under the “housing first” model. I also support the development of client-focused treatment plans and work with clients to establish case plans that address critical needs such as mental health and AOD treatment, education, employment, budgeting, parenting skills and more. I provide crisis intervention, referrals, and collaboratively consult with any service providers working with clients. Overall, as a HUD Case Manager I serve as a role model to guide clients and facilitate appropriate behavior regarding daily living skills, self-care, personal interaction, social relationships and constructive time management.
