I gave support and information to people with dementia and their family/carers. I supported people to get access to a diagnosis of dementia by referring to their GP and Memory Clinic. Support was provided by home visits, office visits, telephone support or post/email and I had a presence in the local Memory clinics at Bassetlaw and Retford hospitals. I facilitated Carer Information sessions over a six week period providing information on Understanding dementia, brain and behavior, unusual behavior, coping strategies, lasting power of attorney and benefits, carers looking after yourselves and introduction to other professionals (social services,CPN's and agencies) and lastly open discussion. All sessions were evaluated and proved to be very informative and beneficial to the carers. I coordinated Dementia Cafes sourcing appropriate venues, activities and speakers, this provided social inclusion for both people with dementia and their family/carers. I helped people understand the different types of dementia, stages of dementia and how they affect the brain and behavior. I provided Information, support and advice on getting a diagnosis, after a diagnosis, coping strategies, respite, future care, benefits, support groups, signposting to other services, Carer’s Assessments, Safeguarding, Lasting Power of Attorney, grief/bereavement, incontinence and much more. I worked in collaboration with other healthcare professionals, consultants, social workers, CPN's and other care providers. I advocated where needed to provide the best outcomes for all involved to prevent crisis situations This role included preparing risk assessments, care plans and record keeping maintaining confidentiality at all times.
