  As Medical Social Worker, one makes weekly or monthly home and facility patient visits for comprehensive assessment. As the LMSW, the responsibilities are assessing and addressing the following, but not limited to: social/psychological/ emotional/bereavement/and physical issues reported by family and or patient. Securing all possible community resources is also an important role of the Medical Social Worker. Some of these may include but are not limited to the DOL application process, VA Aid and Attendance application process, Medicaid application for health care and or Nursing Home placement, securing in-home care givers when appropriate, securing transportation when needed, securing 5 day respite stays at a nursing facility for care-giver respite, addressing and discussing any grief and bereavement issues for Pt and or family. As Volunteer Manager, one is responsible for recruiting, training and supervising all volunteers. Volunteers can either train to work in the office as an admin volunteer, or can train to visit with Patients in their home. Show less
