      We provide support and rehabilitation for clients who have a primary diagnosis of an enduring mental disorder and/ or significant maladaptive behaviours. We provide initial support if their condition makes it difficult for them to function successfully and independently in the community without additional support and rehabilitation. Support for clients who need physically secure accommodation. My role involves managing a team of professionals to support clients with mental health issues, I recognise our role is to develop a good relationship with every client and to understand how best to support them to maximise their independence, quality of life and potential, whilst being in a position to work with them and statutory services to support their physical and mental wellbeing. Our clients’ homes have their own individual areas, a lot of communal space and are designed to promote their rehabilitation and recovery of our clients whilst being homely. All residential care homes are single gender (i.e. all male or all female) and staffed 24 hours per day. Our operations, training and ethos are based upon delivering effective relational and procedural security. I recognise the value of consistency and effective multi-disciplinary working in providing an overall environment in which individuals can make good progress towards their agreed goals I support and mentor the team, running team meetings, supervisions and appraisals.       I provided support for their Grandson who had Pottocki-Shaffer syndrome (a disorder that affects development of the bones, brain, and other organs. Bone tumours form called exostoses. This syndrome effects individuals causing enlarged openings in the two bones that make up much of the top and sides of the skull (enlarged parietal foramina). These abnormal openings form extra "soft spots" on the head, in addition to the two that new-borns normally have.) My role was to give this young person full personal care in a respectful way, to teach him to feed himself and aid with his communication and help with behavioural issue.       Cartrefi Cymru won the contract from Opportunity Housing Trust, so my role was similar to Support Team Manager Role, with the addition of four more services and service users. In April 2012 my role changed and I was asked to swap for a short period of time with the Manager of Carmarthenshire which supported one day service and several sessional services, the manager left after a short period of time so, I was running the Carmarthenshire services and Pembrokeshire services simultaneously, this helped with my time management and communication skills and paperwork skills, the service in Carmarthenshire had no structure or paperwork systems so these had to be started by myself. I was moved permanently to manage Carmarthenshire at the end of 2012; the service grew steadily since my arrival and attained a number of new services after I arrived. The clients I supported have mental, physical and communication difficulties ranging from slight to severe. I also coached a team of support works to improve all aspects of the service and their person centred approach. Show less
