      In this role I work with homeless families with young children who have social, emotional, behavioral challenges or are at risk of a serious emotional disturbance. I provide family centered, family driven care and care coordination to help families reach their short term goals and prepare for their long term goals. I advocate and teach them how to navigate the various systems of care they would become a part of. I also teach and encourage self efficacy skills to promote family voice and choice in decisions that affect them and help ease the transition for families and collateral agencies to ensure that this is a priority.
