  As Program Officer of the Live Healthy Community Partnerships (LHCP) Initiative (a place-based health initiative), I serve as the principal liaison between Health Foundation of South Florida and the two LHCP communities; Miami Gardens and Little Havana. The LHCP initiative, which launched in February 2014, is part of the Foundation’s strategic and systemic effort to reduce poor health outcomes by engaging community residents to improve selected public health indicators. Throughout the initiative, my work has focused on providing guidance, support, and technical assistance to the two communities as they formed and sustained multi-sector leadership councils, selected areas of focus, and developed community action plans with a heavy focus on policy, systems, and environmental change. In addition, I lead grantmaking for the LHCP initiative through a collaborative, co-design process, and oversee monitoring for the portfolio of grants related to the two initiatives. Show less
