AHP2 Rotational Occupational Therapist working on the Acute Stroke Unit.Assessments: - Comprehensive Neurological Assessments including Upper Limb, visual, sensory, perceptual and cognitive assessments as per the National Stroke Foundation guidelines. - Specific tools I use are the MOCA, RUDAS, Addenbrookes Cognitive Assessment, Rivermead Behavioural Memory Test, The Orientation Log, Verbal Reasoning Subtest and The Catherine Bergego Scale. - Functional, kitchen, bathroom and community skills assessments are also utilities to assist with goal setting and discharge planning. Standardised Functional assessments included are, Comprehensive Kitchen Assessment, The Kettle Test, Modified Barthel Index and the Multiple Errands Test.Interventions: - Patient education regarding clinical condition and management and liaising with patient stakeholders regarding goals and therapy plans.- Upper Limb therapy through positioning, strengthening, range of movement and coordination programs, including Electrical stimulation for function and shoulder subluxation prevention and CIMT. - Cognitive retraining and task specific functional retraining. - Pressure injury risk management with cushion prescription and education for patients and family members on prevention. Accomplishments: - Supervision of both more junior Occupational Therapy staff in the Orthopaedic unit and Allied Health Assistants in the Acute Stroke Unit. - Facilitator of The FMC OT Neurosciences Journal Club. - OT Representative of the Allied Health Quality and Safety Team. - Completed a 1 day Stroke rehabilitation course.- Facilitator of The FMC OT Social Club. Interventions: - Independently fabricate a range of Static and Dynamic splints for active or passive management protocols of flexor and extensor tendon repairs, fractures and nerve conditions. - Provision of patient education and development of comprehensive intervention plans for scar management, oedema management and garment prescription, and programs for active, passive or strengthening exercises with foam, putty or functional gripping.- Confident with the management of common conditions such as arthritic conditions, carpal tunnel, Dupuytrens, trigger finger, and nerve conditions such as carpal and cubital tunnel syndromes.
