Assist residents with ADLs such as eating, dressing, bathing, ambulating around their units, etc. Assist residents with dementia, chronic health conditions/diseases, high-risk for falls and elopment, and/or require hospice services. Report patient progression, injuries, and/or requests to supervising LPN, documenting any incidents. Maintain consistent open communication between fellow CNAs, surpervising LPNs and residents to ensure maximum care possible.
