  Clinical Nurse Specialist in the management of Diabetes - Type 1, Type 2, Gestational and Pre-diabetes in adults and adolescents in the acute inpatient and community outpatient settings. This involved consulting with clients in an individual setting, assisting with medication adjustment, initiating technology (E.g. Continuous Glucose Monitor), interpreting reports from technology used with diabetes management, providing group education, working with the multidisciplinary team and assisting with Quality Improvement projects for Inverell Hospital and Community Health Centre.My main achievements include:- Expanding the service to provide group education for people newly diagnosed with diabetes or pre-diabetes- Initial follow-up after hospital discharge after a new diagnosis of Type 1 diabetes.- Provide transition education to adolescents with Type 1 diabetes- Delivering staff in-services to New Graduates and experienced Nurses, including management of Continuous Glucose Monitors, IV insulin infusion, non-insulin medication options, SGLT2i and basal insulins- Implement a Quality Improvement project to ensure all areas of Community Health met their reporting KPIs for the electronic management record (CHIME) used, which was required to secure funding.- Mentored Nursing students and provided Orientation to new staff to the Diabetes Service- Develop new templates for electronic documentation and outgoing correspondence (E.g. discharge Letter to GP) Show less
