  • Work in collaboration with Nurse Case Managers to develop and input care plans which establish goals based on nursing diagnoses and incorporate therapeutic, preventative, and rehabilitative nursing actions • Review patient referral, including history and physical, previous visit notes and other documentation to ensure vital information is incorporated at SOC, Recertification, ROC, and Discharge • Work in collaboration with Nurse Case Managers to create and input accurate medication profiles and to identify any potential complication/interactions • Collaborate with Nurse Case Managers to develop/ create appropriate care summaries • Complete and submit necessary documentation (485, care summaries, orders and medication profiles) in accordance with facility and applicable regulations • Coordinate with Nurse Case Managers to identify appropriate resources to meet patient and/or family needs to facilitate optimal patient outcomes • Function as a team member collaborating with Nurse Case Managers, physicians and Interdisciplinary Team Show less
