  Responsible for the development of functional assessments, behavior support plans, and progress reviews for DDA funded, supported-living programs. Provided assistance to coordinator teams concerning plans needing review, revision, or modification. Provided coaching, guidance, and editing of plans to all members of the support team. Actively reviewed data collection systems to identify trends, and to ensure successful implementation of Positive Behavior Support Plans. Coordinated with support teams to meet desired outcomes. Individual and group training of employees on the use of Positive Behavior Support plans. Respond to crisis situations; assisted team by providing alternative techniques, modeling and training with physical interventions. Served as advocate for supported individuals and maintained routine communication with families, healthcare professionals, and other supports in the individual’s life. Provided direction, oversight, training, and feedback to staff regarding supporting individuals per defined treatment/support plans.
