I address the broad array of my client needs through referral to providers of necessary services. As an Homeless Service Specialist I act primarily as a care coordinator than a provider of direct services,however, I may perform duties that include outreach, prevention and harm reduction, crisis intervention, health education, substance abuse and mental health referrals, and advocate for benefit services. As a Housing Program Specialist/Advocate and Case Manager, I reach out to governmental, faith based, nonprofit service providers and local business to continue building upon and towards the ultimate goal to develop a cohesive, collaborative and coordinated system of care to create a seamless, client centered system of case management, providing the capacity to extend the reach of resources available to the City of Madison's homeless population. I help to facilitate and coordinate supportive services with various partners, including employment assistance programs, job readiness training, disability resources and financial education. I Interview potential candidate to determine if the client qualifies for Housing First, HOPE, ReachOut Housing First or the PHP program. I assist in the coordination of a smooth transaction for the client to transition from homelessness to housing through developing a plan for wraparound services according to the client's needs. I collect and enter data into HMIS for supportive services and reporting. I have also (very recently) received completed my classroom hours and my service learning hours, passed the State exam, and am a Substance Abuse Counselor in Training. I am also a recent graduate from Madison College, and a member of Phi-Theta Kappa international honor society for academic excellence. I completed my studies on May 13th, 2016 and obtained an Associate Degree in Applied Science(Major) and (minor) Human Services. Show less
