  I am the State Designated Permanency Planning Coordinator for individuals under the age of 22 living in nursing facilities in the State of Texas. I complete the state mandated residential goals. I discuss with families the services/supports available that will enable them to bring their child home or to a family based environment near them. For those living in nursing facilities these supports/services provided by Federal Medicaid Waiver programs are automatically eligible to them, there is no wait. Money Follows The Person (HCS, CLASS, MDCP, STAR PLUS) Waiver programs will do things like: nursing, attendant care, home modifications, transportation services and much more. Since transitioning back home is a process, EveryChild coordinates the entire transition. Everything is in place before the child returns back to a family based environment. I will follow up and help with any issues/barriers once the child returns home. AT RISK YOUTH: I also assist children at risk of a nursing facility or State Supported Living Center: I request and advocate for the services needed so children can remain at home with supports. EveryChild advocates for the families and the children .Learning from families and using their experience toward making changes on a legislative level.
