- Coordinate grant-funded projects on individual and group wellness programs aimed at educating and encouraging vulnerable communities to adopt evidence-based healthy lifestyles to prevent type 2 diabetes, heart disease, hypertension, obesity, and cancer. - Perform research on relevant topics in health promotion and disease prevention and conduct surveys to meet program participant’s needs and address the social determinants of health. - Build effective relationships, identify expectations, and offer practical, beneficial recommendations and solutions to identified challenges, provide written reporting of patient engagement. - Coordinate care with other health-related partners such as healthcare providers, personal trainers, physical therapists, and Registered Dietitians for optimal care. - Reach out and maintain an ongoing relationship with community partners and other stakeholders to drive the organization’s goals. - Develop and conduct health and wellness seminars and training courses for adult learners of different size groups in various settings. - Design and distribute education materials and newsletters, and cultivate an online social media presence. - Pursue continuing education in wellness related fields including fitness, positive psychology, coaching, disease prevention and treatment, dietary theories and progressive wellness approaches.
