  I contact current UDS waiver participants annually and newly enrolled waiver participants. By asking strategic questions I am able to discover problems,affecting the participants services and well being and do the follow up on these issues with the service coordinators and agencies. I compile reports for the Quality Assurance Committee about the Service Coordinators, attendants, attendant care agencies, services, attendant training and numerical ratings of service and agency satisfaction. We then review comments/ complaints and the follow up that was done and the results of the follow up. Ratings are followed and when a trend is noticied it is discussed and follow up is then decided for better service.Currently the surveys are available in English and Spanish either by phone or mail.   I coordinated the visits and follow ups of 6 Peer Counselors for the State's Exceptional Durable Medical Equipment Program. UDS received a list of approximately 500 residents in Nursing Facilities in about 8 counties who needed to be visited within 1 year. The visits were to determine if they had the equipment they needed or were they in need of new or updated equipment. This equipment could be a new power chair, a communication device, any assistive device as long as it was over a minimum of $5000. The state would then consider this as exceptional and after review would determine if it should be purchased. I kept statistics which were reported on a monthly basis to the state and the program director at UDS. I fiscally managed a given budget and made sure the Peer Counselors received payment for their job specifications and expenses. Show less
