  Current Role : Primary Care provider with longstanding practice in Adult Medicine . This is my primary role and one I have found to be quite rewarding . Work in a collaborative relationship and developing "Team Based Care" model. Responsible for the routine , acute and chronic health care needs of patients. January 2014. Assigned to promote the services of AP's amongst a multispecialty group practice. AP Leadership : Initiated LEAN process to streamline provider onboarding Assisted in the development of AP residency program Participant in the PCIS (Quality Committee) Participant in LEAN process including my own practice -building Team Based Care.
