This role includes the management of a case load of ADHC clients with disability and complex needs. This includes home visits, facilitating and chairing case conferences, organising support, services and respite for the individual clients and their families/carers. This role also includes preparing clients for their transition to NDIS. As part of this role I have:• Effectively manage a case load of approximately 16 people with disability and complex needs under a person centred framework.• Contribute to the development of evidence based high quality social policy i.e. Abuse and Neglect Policy, Risk and Safety Policy, Guardianship Policy, Lifestyle Planning Policy…• Working with stakeholders on the Restrictive Practices ‘Psychoactive Medication Review Project’.• Develop case plans, behavioural assessments and client reports.• Chair and coordinate case conferences with clients and their families / carers and other service providers.• Effectively work in a multi-disciplinary team.• Maintain strong interagency networks to support collaborative case management practices.• Support clients to understand and prepare for the transition to NDIS.• Maintain client files and correspondence.• Provided court reports and provide files when subpoenaed.• Write up Briefs for Short Term Funding Submissions.
