      In this position I perform a broad range of counseling, case management, supervisory, and administrative functions. I direct the provision of services to clients on their assigned case load, and supervise 3 Mental Health Coordinators. I perform case management duties: Completes documentation including, but not limited to, the following: functional assessment, individual community support plan, case notes, supervision and training records Obtains diagnostic assessments and side effects assessments as required. Orients assigned clients to case management services and rights. Ensures client involvement in the development and review of the individual community plan. Maintains regular contact and communication with assigned clients.Reviews assigned clients’ progress toward goals, amending the service plan as needed to maximize client progress.Refers and assists assigned clients in obtaining needed mental health and other services, ensuring that all needed services are secured and provided in a timely manner, and inform supervisor if services are not provided as needed.Maintains communication with the client’s family, other service providers, legal representative, or other relevant persons to ensure coordination of services. Supervises assigned program coordinators: Supervises and reviews the performance of assigned employees. Makes recommendations concerning the hiring and termination of the above to the Executive Director. Directs and supervises supervisees' performance using a variety of techniques including, but not limited, to:a. Regularly scheduled one-to-one supervisory conferences;b. Frequent informal/unstructured observation, with feedback loop;c. Hands-on demonstration of desired skills, role playing of situations, etc.* I currently oversee 35 clients, 19 of which are case management.
