Obtains and documents vital signs & weight accurately and reports abnormal results to the nurse; documents appropriately on the electronic medical record. Assists with admission, discharge, and transferring of patients. Updates hospital tracking system to reflect patient transfer. Provides assistance for personal care to patients including bathing, dressing, dietary needs, and toileting. Ensures security of patient’s personal items and reports discrepancies to supervisor in a timely fashion. Assists nurse and/or physician with implementing treatment interventions (i.e. sterile and non-sterile soaks and/or dressings). Documents patient intake/output, sends urine specimens to labs as ordered, performs daily phlebotomy as ordered, collects and labels tubes properly using hospital protocol and performs bedside glucose monitoring as instructed by nurse. Responds to patient call light in a timely manner; assists patients with moving, turning, getting out of bed and ambulation. Provides for patient safety through preventative measures (bed controls, nurse/patient communication system, side rails).
