• Joined the multidisciplinary, specialist, MND service, at its inception and contributed to the continual growth, redesign and development of the service to meet increasing consumer demand.• Coordinated the delivery of high-quality, person-centred care, for >500 patients with MND:− Providing expert consultative advice on MND management to patients, carers and healthcare professionals; engaging relevant medical specialists, allied health, and nurses, to ensure holistic, integrated support.− Embedded robust clinical indicators and frameworks to regularly assess and monitor patients’ disease progression and symptoms, using a variety of outcome measures and service delivery methods (such as clinics, telehealth, inpatient admissions and home visits).− Transformed the Advance Care Planning process in the service.• Nurtured positive and productive relationships with multidisciplinary stakeholders throughout the Hospitals and community health network, communicating actively and advocating for the needs of the patient to ensure a shared commitment to quality outcomes.• Navigated the service and the patients through the impact of COVID-19, rapidly pivoting to remote services that optimised telehealth solutions.• Built rapport, trust and credibility with patients from diverse cultural and socioeconomic backgrounds, at times of crisis, demonstrating empathy, compassion and expert knowledge.• Championed innovation and improvement, leading numerous clinical research and quality improvement projects.
