      I Worked full-time in adult physical rehab inpatient unit. Assessment of each patient and working both individually and through facilitating therapeutic groups offered on the unit. Collaborating and communication with other disciplines (example: physical therapy, occupational therapy, speech therapy and psychology). Utilizing EPIC online documentation system to write progress notes after each session. Documentation is compressed of the following: evaluation, progress notes, re-evaluation notes on patients 3x/week and lastly discharge notes. Consistent collaboration and communication with each member of treatment team, as necessary. Involvement and additional support provided to patient's family as appropriate and necessary.
