This position involves working with young people between the ages of 16 to 18 years of age in a specialist environment. The 16+ Service aims is a client centred service that works using the pathway plan assessment to plan the transitional journey to adulthood. This includes but not limited to: • Emotional and Behavioural Support (E.g. Anger Management, Therapy, Parenting/Relationship Support) • Contact Arrangements (with families / significant others) • Accommodation / Housing options, advice & guidance • Looking at healthy living and lifestyles. • Promoting Psychological Health (directly and indirectly) • Assistance with social networks and relationships • Positive activities • Education Training & Employment (ETE) • Financial arrangements as under the Act for example setting up home allowance, or bursary for higher education. • Independent Living Skills • Substance Misuse Issues • Group work (Art, cooking, football) . Sexual Health awareness and support to access. The 16 plus service works with Looked After Children (LAC) aged 16 to 18 years of age but depending on individual circustances some young 16 plus continues to support them up to the age of 25 years if still in education or training. The service also works with Unaccompanied Asylum Seeking Children (UASC) as this area of work requires some particular skills and knowledge the service works with all UASC even if younger than 16. At the end of October, 2017 the 16 plus service was supporting 407 young people this number is comprised of: • 170 Looked After Children • 237 Care Leavers • Within the 407 we have 73 UASC. In my role a Support Worker i provide positive support to the clients by challenging barriers including substance misuse, domestic violence, self-harm amongst others aiming to empower clients to develop the skills needed to live independently. As a Support Worker i work with the clients and others close to the client to develop individual personalised support packages reflective of their needs through keyworking to deliver best practice in crisis intervention and while delivering planned support. I undertake referrals and assessment of potential clients to identify their needs producing support package from this. Community Family Worker within a Family Services Team working to ensure all children get the best possible start in line with the Every Child Matters (ECM) agenda in a multi disciplinary approach engaging with other organisation such as social services, health professionals, the police and learning institutions among others. Child protection being an integral part of this job required that workers are able to recognise signs and symptoms of non-accidental injury, neglect, sexual, physical or emotional abuse taking appropriate steps where abuse is identified as stipulated by the National Safeguarding Board. Prepare reports on safe guarding issues and participating in safeguarding conferences. Working in the community with disadvantaged groups with children delivering a range of parenting interventions while helping them reintegrate back into society by assisting in skill development and signposting to various sites or organizations that can help. with a small budget i organised promotional activities where service users showcased their skills to prospective employers and also fun activies for families. This position also has managerial responsibility of the F grade Community Family Workers and centre volunteers addressing issues at supervision meeting and ensuring progress and development through appraisals.
