      This position involves working with young people between the ages of 16 to 18 years of age in a specialist environment. The 16+ Service aims is a client centred service that works using the pathway plan assessment to plan the transitional journey to adulthood. This includes but not limited to: • Emotional and Behavioural Support (E.g. Anger Management, Therapy, Parenting/Relationship Support) • Contact Arrangements (with families / significant others) • Accommodation / Housing options, advice & guidance • Looking at healthy living and lifestyles. • Promoting Psychological Health (directly and indirectly) • Assistance with social networks and relationships • Positive activities • Education Training & Employment (ETE) • Financial arrangements as under the Act for example setting up home allowance, or bursary for higher education. • Independent Living Skills • Substance Misuse Issues • Group work (Art, cooking, football) . Sexual Health awareness and support to access. The 16 plus service works with Looked After Children (LAC) aged 16 to 18 years of age but depending on individual circustances some young 16 plus continues to support them up to the age of 25 years if still in education or training. The service also works with Unaccompanied Asylum Seeking Children (UASC) as this area of work requires some particular skills and knowledge the service works with all UASC even if younger than 16. At the end of October, 2017 the 16 plus service was supporting 407 young people this number is comprised of: • 170 Looked After Children • 237 Care Leavers • Within the 407 we have 73 UASC.
