      I led Geriatric and Extended Care programs in healthcare systems for Veterans in NC, VA, and WV. Programs included nursing homes, home care, palliative care, and outpatient clinics for frail and/or disabled individuals and oversaw expansion of access to specialty geriatric evaluation, home based primary care, adult day health care, homemaker home health aide services, and hospice and palliative care. I supported the establishment of new care models for transitions from hospital to home. I employed telemedicine approaches for home care and supported rural providers. I supervised efforts to change institutional culture transitioning from nursing homes to community living centers. I was responsible for oversight of system wide efforts to reduce use of antipsychotics, indwelling catheters, and restraints. I promoted the use of therapeutic and meaningful activities in institutional settings. During this period, I also served as Systems Redesign Lead point of contact for VHA health care systems in the network and promoted lean management. I acted as Physician Representative for Survey Readiness Team and conducted annual facility audits to ensure compliance with Joint Commission, VHA, OSHA, and other industry standards.
