I am worked as a Spiritual Care Coordinator at Gentiva/Kindred Hospice. I visited patients in their homes, ALFs, AFCs, Independent Living, and hospitals to help them with their spiritual needs. I also developed a spiritual Plan of Care with them and their families, participated in interdisciplinary team and employee engagement meetings each week, and filled out visit notes and Comprehensive Spiritual Assessments each visit for current and new patients respectively.
