      As part of a collaboration between San Francisco Collaborative Courts and the Tenderloin Housing Clinic, my role as the assigned Transitional Housing Planning Specialist for the "Collaborative Courts Emergency Stabilization Housing" program, is to support our clients (most of whom are often on probation and were homeless upon intake) with providing them with emergency housing in an SRO, over a 6-Month period, per requirements of this Adult Probation Department (APD) funded program.My position entails that I meet with my clients often to monitor their progress towards finding long-term housing. This includes weekly visits on-site at their SRO to perform room inspections to ensure that clients are in compliance with the program requirements, such as being able to demonstrate that they can maintain a clean living environment.Many of our clients deal with various mental health issues, including PTSD from being incarcerated and being homeless, and/or a dual-diagnosis. While reinforcing the Harm Reduction Model approach of the Tenderloin Housing Clinic, I support my clients by linking them up to Support Services and communicate with their Case Managers and/or Probation Officers, amongst others, to help address the obstacles that they face with finding long-term housing.
