Serve youth and families of Baker, Bradford, parts of St. Johns, and Union Counties in locating, identifying, screening, and referring children at risk of disability for evaluation for special education / related services. Facilitate screenings through telephone conferences, childcare centers, community settings, or generally accessible locations. Perform case management and service planning activities to ensure children and families receive referrals that link to the local school district and other community agencies and organizations.
