The Transforming Care Programme aimed to improve services for people with learning disabilities and/or autism, who display behaviour that challenges, including those with a mental health condition. The objectives aimed to reduce the reliance on inpatient care, by re-shaping local services to meet individual’s needs in the community where it is appropriate to do so. I was responsible for developing, managing and reporting on the regional project plans so that local commissioners were able to develop effective commissioning strategies for the transformation of learning disability services. Working collaboratively with key stakeholders and system partners across the NHS England matrix and wider Health and Social Care system to develop and share best practice. Monitoring progress against trajectories and capacity plans, creating strategic reports/briefings for Senior Management Team and Regional Transforming Care Board.Operating professionally in a highly political and sensitive environment whilst managing the project team and driving delivery of a range of business initiatives and projects. Contributing to the strategic planning of team projects, identifying inter-dependencies across projects/functions, potential impacts on the wider organisation, resource requirements and building in contingency and adjustments as necessary.
