Back on Track is for young adults typically ages 18-30 who are facing their first felony charge involving a low-level drug offense. To be eligible, defendants cannot have any history of violence, guns or gang involvement. The program starts in court when a defendant is given a choice to join Back On Track or stand trial. If a defendant chooses Back On Track, to get into the program he must first plead guilty and perform up to 220 hours of community service. Formal sentencing is then deferred for 12 months while he completes a rigorous, year-long Personal Responsibility Plan that mandates concrete goals to be completed, such as getting a G.E.D. or finishing high school, holding down full-time employment, completing parenting classes, and paying child support obligations.• Conducted initial orientation and intakes for all participants who are referred to the Program• Enter all initial data into Metsys, Client Track systems & compose files for each referral obtained by the courts• Document participants’ progress and maintain appropriate documentation in files & case notes per CARF standards• Attend all initial court dates and all DEJ court dates for each referred participant while in the Pre phase• Attend & partake in weekly case conferencing meetings with BOT, FSA and DA staff• Organize and submit biweekly evaluations and progress reports documenting client progress to Superior Court• Assist in organizing monthly meetings & keeping attendance• Attend Community Based Organization (CBO) meetings while developing relationships with outside partners for the program, making referrals to outside services, making referrals to internal services• Facilitate weekly client in-office check ins while assisting participants in developing and carrying out their individualized Personal Responsibility Plans• Composing participant data for individual and program monthly reports and manage on-going master lists Show less
