I complete pharmacy consults throughout the 1000-bed hospital, including dosing medications according to renal function, antibiotics dosing, DOAC monitoring, pharmacokinetics/pharmacodynamic monitoring ect. I also take care of all anticoagulation management of the entire hospital, which include new initiation of heparin (including mechanical circulatory devices) and heparin rate adjustment and monitoring per PTT/Anti-xa results. In addition, I am responsible to clarify clinical questions from the operational site (e.g., technicians and staff pharmacists)s as well as questions from other healthcare professionals. During my shift, I respond to emergency situations such as code blue and code stroke or situations that require pharmacy assistance. Show less
