  My roles and responsibilities comprise of: Assisting the Care Manager with the admission process of new residents by performing a psychosocial assessment to determine the level of care required and to assess whether specific services such as counselling or psychiatric interventions are needed. Discussing findings of psychosocial assessments with the Multi-Disciplinary Team and collaborate with team to formulate a treatment plan for each resident. Assisting residents to adjust to their life in their new homes and living environment by providing individual or group counselling as needed to overcome potential emotional, psychological, and social obstacles to adjustment. Planning and implementation of wellness programme for residents which involves designing engagement programs for all residents focusing on physical exercise, stress reduction, cognitive stimulation and social interventions Educating all staff about the psychosocial needs of residents and providing training on multicultural and diversity issues.   During this time, I worked within a multidisciplinary team of professionals. My roles and responsibilities were comprised of: Completing a psycho-social assessment with the patient and families Arranging and facilitating family meetings with the doctor, nurses, therapists and family. Providing supportive counselling to assist all involved to come to terms with the patient’s disability as well safe discharge planning Providing resources pertaining to community support agencies to assist them in reintegrating into their family system, home environment, community and work place Facilitating work visits and employer liaison as well as empowerment of patients when liaising with government departments e.g. The department of Labour. The focus of the above is on supporting the patients and their families in regaining their independence after sustaining a disability and experiencing a loss.
