Youth In Transition Worker
-
Mar 2022 - Present
Develop, work on and maintain independent living skills (ex. cleaning, cooking, and budgeting). Builds confidence in a youth’s own ability to do these things. Connect with community resources, groups and places where social networks can grow for the youth. Familiarity with one’s community helps foster inclusion and eliminates isolation Act as a mentor / supportive adult in the community for young people who need social connections. This helps alleviate social isolation when living alone. Provide support and advocacy in navigating systems when living independently (landlords, health related issues, benefits / insurance, etc.). Can often times be complex causing frustration and “giving up” so being able to do this enhances a youth’s ability to learn how to do this in future. Liaise care and reduce various forms of risk with multiple agencies (CAS/CCAS/other shelters/health care providers etc.) to establish roles and avoid service duplication. Often times youth are involved with myriad organization and care providers; having open communication and establishing roles in care provision is important, particularly if the youth may not have the capacity to do this on their own. Guide and lead youth to identify, develop, work on and maintain their own identified goals Can be anything they identify, providing support can foster resilience and quality of life.
