  HCRs act as advocates and meet with patients and their families daily. Detailed Responsibilities: •Work effectively with hospital employees and patients. •Interviewing clients in a hospital setting and/or home visits to assist the clients in obtaining maximum benefit coverage. •Completion of applications for state and federal programs (including Health Exchange/Marketplace programs), reviewing medical records and taking all necessary action to expedite benefit approval. •Provide updates and assistance to hospital personnel and other Chamberlin Edmonds staff as needed. •Making a determination to take a Federal, State or Local program application based upon medical definition of disability as it relates to body systems and to functions of daily living, and information contained in medical records. •Knowledge of Florida Medicaid and/or SSI/SSDI disability application processes and programs •Ability to read and apply the information contained in medical records to appropriate program •Previous case management experience •Reliable transportation; home visits within the local area will be necessary •Previous experience in a medical/clinical/hospital setting •Demonstrated experience working with people of various educational and socioeconomic backgrounds •Computer skills, including word processing and data entry, (MS Office, Outlook, Excel, preferred) •Demonstrated strong written and verbal communication skills PREFERRED: •Bachelor’s Degree, preferably in Social Work, Nursing or Psychology •Fluency in either Spanish, Haitian Creole or Portuguese preferred •Previous experience in Social Work, Nursing and/or Utilization Review •Previous Hospital business office experience in Financial Counseling or Admissions •Previous experience working for Social Security Administration (SSA), Disability Determination Services (DDS), Department of Children and Family (DCF) and/or Agency for Health Care Administration •ACA Certified Application Counselor Show less
